Blood test

If your physician from Trois Lacs medical center prescribed a blood test, you can do it in a CLSC or a private place; your doctor is going to receive the results either way.

Forms

If you need your physician to complete a form (SAAQ, insurance or else), you can fax it or bring it to the center. Your physician has one month to complete it, but he/she will try to do it faster. You might be charged a small fee for the completion of the form, as indicated in this table of the FMOQ*.

*If a patient thinks that the amount payed is for a service covered by the health insurance Law, he/she can write within the year following payment, to ask for a reimbursement by the Régie de l’assurance maladie du Québec, who will reimburse the amount, when they think that the patient wasn’t supposed to pay, they will take the said amount from the physicians or third party involved.

Getting ready for your appointment

On the day of your appointment, it is important to take your medical insurance card and a list of the medicine you are currently taking.

Nursing Care

Our nurses are available to vaccinate, do screening of sexually transmitted and blood-borne infections (STBBI), diabetes and arterial blood pressure follow-up, talk and help you quit smoking, help you if you breastfeed your baby, and more. Feel free to contact one of them if you are already a patient of the clinic.

Prescription/Refill

If you lost your prescription, you can contact the secretary of the center. She might have to give you another appointment.

For a prescription refill of your medicine, you will normally need an appointment. Don’t wait until the end of your treatment to ask for a refill, contact us in advance. Your pharmacist might be able to help you before your next appointment, he can also ask your physician for a refill via fax.

In the case that your doctor rewrites a lost prescription or does a refill without an appointment, you might be charged a fee, according to the table of the FMOQ*.

If you need a reference to see a specialist or another health care professional, it is best to talk to your general practitioner about it during your appointment. The doctor might agree to this without an appointment, if it is regarding a problem you already talked about, but he might prefer to talk about it in person. You can ask the secretary of the health center, she will follow-up with your physician and let you know.
